*Resident Care Manager
(4 rotating shift positions available)
Position Overview:
The Residential Care Manager (RM) is a critical member of the medical respite care team. The RCM provides evening and overnight support and assistance to residents in medical respite care. RCM care includes responding to resident medical respite core service needs during shift hours and ensuring environmental and resident safety.
Qualifications:
High school education or GED required. Previous direct experience with the homeless population preferred, or previous experience working with underserved populations preferred. Lived experience welcomed.
General:
- This is an overnight shift position in which the RCC remains awake to ensure the safety/supervision of residents
- Complies with policies and procedures developed by SSM
- Must be able to get to and from SSM in a variety of weather conditions
- May require occasional resident transportation to medical appointments
- Other duties could include light housekeeping, data entry, light office work
*Medical Respite Case Manager
Position Overview:
The Case Manager (CM) is a critical member of the respite care team. The CM is responsible for assessing the needs of residents requiring temporary medical respite care, developing individualized care plans, coordinating necessary services to improve health and social outcomes, discharge planning, and advocating for resident needs. This involves close collaboration with community partner providers, hospitals, agencies and other stakeholders to ensure further care upon discharge from respite.
Qualifications:
Education Requirements: Bachelor’s degree in social work (licensed), psychology, or related field. Previous direct experience with the homeless population preferred, or previous experience working with underserved populations preferred.
